In this vital role you'll deliver person-centred, high-quality support to tenants—helping them achieve their personal goals and improve their wellbeing across social, health, and recovery domains.You'll also play a key role in supporting tenants with complex needs, offering hands-on guidance and connecting them with the right services and resources.
As part of our mobile Tailored Support Coordination team, you'll have the opportunity to make a difference and travel across multiple housing communities.
This role will be suited to someone who is adaptable, flexible, passionate about Affordable Housing and willing to learn and grow in this field.This position is Permanent Part time, 4 days per week.
Your position will require you to provide 1-day direct service delivery at Anglicare Boroni (Fairfield) and manage a case load of 26 SAHF residents.
3 remaining days will require you to provide support to other housing sites including Arista (Mt Druitt) and Calli (Liverpool) or assist with other operational requirements as per the Senior Program Manager's directive.
Anglicare office location base will be discussed with the successful candidate.
What You'll Do:Ensure that residents engage with the services identified in their Support Plan by facilitating resident access to support services through such activities as coordinating transportation to appointments, providing appointment reminders, attending important meetings with the Residents.Ensure positive outcomes for Residents by establishing and maintaining strong working relationships with service providersEnsure support is meeting needs of Residents by actively soliciting and documenting feedback regarding the suitability and quality of the programs and services that they receive.Ensure Support Plan continues to meet identified needs of residents by maintaining regular contact with service providersAssistance in the management of Tenants and Household members with complex needsManagement and administration of financial and operating performanceDeveloping strong relationships with family members, carers and social networks.What You'll Bring:A qualification/degree/certificate/diploma in Social Work, Community Services (Case Management stream), Allied Health, Aged Care, Community Housing, or equivalent, and/or relevant job experience.A minimum of 12 months post-qualifying experience in the delivery of community based social services, including assessment and care coordination.A passion for helping people in crisis or experiencing difficult times in the spirit of compassion, tolerance, empathy and non-judgement.A current driver's licence.Why Anglicare?
