The successful candidate will coordinate, manage, and administer the delivery of care services to our clients.This role is responsible for:Conducting client care assessments and care planningManaging client compliance and a case load of clients.Intake processing, clinical care planning and reportingCoordinate the provision of quality person centred care according to the client's needs, service/care plans and available funding in accordance with budgetGeneral administration with respect to tasks performedOn Call with a rotating roster between the care management teamAssisting with the enquires via phone and walk in enquiresIf you would like to work for a fast growing and industry leading company, then please apply for this position.

Genuine career progression is available as is a very generous remuneration package for the outstanding candidate.QualificationsMinimum of Certificate 3 in Aged Care/Individual Support (Required)Driver Licence (Required)Preferred SkillsPrevious experience within the aged care/NDIS/mental health Care coordination roleKnowledge of My Aged Care, Home Care Package and NDISEnrolled Nurse or Registered Nurse will be an advantageExperienced in assessing and planning care needs within the communityValid Driver's LicenceHigh level of computer literacy, with a sound understanding of Microsoft OfficeAn outgoing personality with high energy levels, good communication skills and personable telephone mannerAbility to problem solve and think quickly under pressurePossess a 'can-do' positive attitude and work well in a team environmentAbility to multitask with a strong attention to detail while keeping calm and focusedExcellent networking, consultation skills to develop new referral sources and relationships with appropriate external organisationsExcellent customer service skills with exceptional verbal and written communication skillsSound knowledge and understanding of home care services and guidelines
