**Project Lead Community Services Improvement Focus**

- The Royal Melbourne Hospital provides a number of community services to support people to leave hospital and stay well in the community. This includes the Aged Care Assessment Service (ACAS), the Transition Care Program - Home Based (TCP Home Based) and the Health Independence Program (HIP). HIP includes the Hospital Admission Risk Program (HARP), Community Therapy Service (CTS), Post-Acute Care (PAC) and Sub-acute Ambulatory Care Services (SACS) clinics.

**Your new role**:

- Responsible for the design and implementation of project activities including problem definition, measurement, analysis, implementation and evaluation
- Work with the Community Services Management team to ensure changes are embedded with standardised and sustainable systems and processes
- Development of training materials and procedures, including consultation with relevant teams to inform requirements
- Develop, plan and implement improvement projects to optimise the delivery of care to our patients
- Projects will focus on improving access, efficiencies, sustainability and quality of care

**About you**:

- A comprehensive understanding of hospital operations, community services and non-admitted funding models
- Demonstrated experience of managing change in a complex environment
- Excellent communication skills including well-developed presentation and writing skills and an ability to promptly prepare relevant documentation, presentations and reports
