In the Northern Territory, we deliver Financial Wellbeing services across Central Australia and are proud to be leading the new Central Intake Service, a Territory-wide initiative improving access to support for people experiencing, or at risk of, homelessness. 

Our work in the NT builds on our strong foundation in South Australia, where we provide services such as foster care, homelessness support, emergency relief, family support, counselling and allied health services. About the Role

At Lutheran Care, we believe the way we support people shapes the impact we have in the community. We're looking for compassionate Pathways Workers who thrive on making meaningful connections and helping individuals and families access safe, respectful, and culturally responsive support. 

As a Pathways Worker, you'll be on the frontline meeting people where they are, hearing their stories, and helping them find the right support. You'll work closely with your Team Leader, collaborate with other service providers, and be part of a connected team that puts people first. 

At the heart of the Central Intake Service is a commitment to being inclusive, respectful, and continually learning through ongoing cultural awareness and understanding. You'll work in close partnership with Yilli Rreung Aboriginal Corporation, The Salvation Army, and other community organisations to make sure services are delivered in a way that is safe, culturally responsive, and meets local needs.

Key Responsibilities:

  • Be the first point of contact for people seeking support, through phone and in-person services.

  • Use assessment tools to understand each person's risks and support needs and connect them with the services that best meet those needs.

  • Support clients to make informed decisions about their options.

  • Develop flexible case plans based on individual goals and backgrounds.

  • Maintain accurate, confidential, and respectful client records.
