Support Coordinator

We are hiring a Support Coordinator to join The Housing Connection on a part-time basis for 2 days a week.
Responsibilities
- You'll work closely with NDIS participants to achieve their goals, ensuring they receive the right supports to thrive.
- This role offers flexibility, autonomy, and the opportunity to make a meaningful contribution to the community.
- Join our dynamic team and help individuals navigate their NDIS plan and achieve their potential.
- Connect participants with appropriate supports, services, and resources.
- Assist participants in understanding and implementing their NDIS plan.
- Maintain strong communication with participants, families, and other stakeholders.
- Accurately record and report on support coordination activities.
- Represent the best interests of NDIS participants, ensuring their needs and preferences are met.
- We are seeking a part-time employee for two days a week. Depending on performance and organisation needs, there is an opportunity for increasing in work days.
Qualifications
- Proven experience in support coordination or a related field.
- Strong communication and interpersonal skills.
- Excellent organizational and problem-solving abilities.
- Ability to work independently and as part of a team.
- Passion for working with people with disabilities.
- Relevant IT skills to ensure adequate skill in record keeping, report writing, NDIS portal access and information interpretation.
- This role is based in Chatswood, NSW.
- Some travel to client locations may be required.
