About the role:
The Care Coordinators are responsible for supporting clients within the Integrated Team Care (ITC) Program and to provide culturally appropriate and holistic primary health care for Aboriginal and Torres Strait Islander communities of Cairns and surrounding districts, including education and promotion, development and implementation of early intervention strategies and clinical screening protocols.
The position will contribute to the delivery of primary health care services incorporating the development, implementation and evaluation of program resources and strategies to meet the needs of Aboriginal and Torres Strait Islander clients, including liaison with relevant community groups, health professionals, organisations and agencies.
The Care Coordinator is supervised by the Coordinator, ITC and the position has no supervisory responsibilities.
This position works closely with other members of the ITC Program and maintains regular contact with other program staff to ensure effective outcomes for clients.
Key responsibilities:
- Conduct comprehensive, culturally competent health services, incorporating promotion, prevention, early intervention, health assessments and treatment in accordance with medical and established Aboriginal and Torres Strait Islander community protocols.
- Undertake client recalls and internal and external referrals in accordance with approved processes, including case coordination activities.
- Undertake crisis intervention, advocacy and referral to appropriate internal and external providers.
- Ensure client records in Communicare are kept current to reflect eligibility for internal and external services, including the Care Coordination and Supplementary Services program, in accordance with approved processes.
- Process new and maintain current ITC registration. Monitor of GPMP to ensure they are up to date and client's needs for ongoing support, follow up with GP to ensure all documentation is received to be able to process in a timely manner.
- Process ITC purchasing request from GP's for services to be purchased and/or hired medical equipment or services. Source Suppliers and quotes for clients, organise delivery of services and medical aids to clients eligible for supplementary services in conjunction with the Outreach Worker
- Provide education to clients and family on the use and maintenance of the medical aid and/or specialist's appointments
- Work in partnership with other health care professionals to ensure a multi-disciplinary approach to comprehensive primary health care
- Participate in organisational and community activities to advance the health and wellbeing of Aboriginal and Torres Strait Islander people.
- Work within Wuchopperen's Policies and Procedures including the Staff Code of Conduct and the Work Health and Safety requirements and contribute to continuous improvement processes to ensure compliance with relevant standards and safe, effective service delivery.

For applicants to be successful in this role, they should demonstrate:
- Demonstrated ability to perform clinical screening, health education and promotion and administrative duties within a clinic setting .
- Extensively developed interpersonal and communication skills including the ability liaise with a range of stakeholders as well as effectively advocate for Aboriginal and Torres Strait Islander people.
- Demonstrated ability to work independently and in a multi-disciplinary team environment, prioritise and meet deadlines, deal with matters of a sensitive and confidential nature.
- Demonstrated ability to meet and work within the core values and behaviours of Wuchopperen
Qualifications and experience:
- Certificate IV or higher in Aboriginal and/or Torres Strait Islander Primary Health Care (Practice) and has a minimum of three (3) years of direct experience in the delivery of clinical health care services.
- Knowledge and understanding of Aboriginal and Torres Strait Islander societies and cultures from an historical and contemporary perspective, including the ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander people.
Requirements:
- It is a condition of employment in this role for the employee to be, and remain, vaccinated against measles, mumps, rubella, varicella (chicken pox), pertussis (whooping cough) and hepatitis B
- It is recommended for the employee to be vaccinated against COVID-19.
- The Care Coordinator must have current registration with the Australian Health Practitioners Regulation Agency and preferably with Immunisation Endorsement.
- Proof of qualifications and current registration is required to be provided prior to the commencement of duty.
- The Care Coordinator is an Identified position. It is a genuine occupational requirement that an Identified position be filled by an Aboriginal person or Torres Strait Islander person, as permitted by and arguable under section 25, 104 and 105 of the Queensland Anti-Discrimination Act
- It is a condition of employment that all employees are holders of a current and valid 'blue card' or Working with Children Check. This is at your cost. Employment cannot continue or commence if you do not have a 'blue card' or if your 'blue card' is withdrawn or is disqualified.
- Successful applicant to consent to a broader Criminal history check
- Possession of a current Queensland "C" class driver's licence is essential.
- Commitment to work within the principles of Aboriginal and Torres Strait Islander community control including a willingness to develop the leadership of Aboriginal and Torres Strait Islander staff.
- All employees are responsible for the safety of themselves, students, visitors, volunteers and fellow staff members. Employees must report any hazards or incidents, co-operate in emergency drills, undertake Workplace H&S training as required, and lodge risk assessments for all external and internal risk activities.
- Ability to use Microsoft products and the ability to use or willingness to gain proficiency in the client information management systems.
- A commitment to adhere to Wuchopperen's policies and procedures.
