About the role:
The Home Support Worker will provide frail, older people, registered with the Commonwealth Home Support program (CHSP), with assistance with transportation and domestic chores, as allocated, to maintain their capacity to manage everyday activities in a safe, secure and healthy home environment.
The hours of support required for each client will be on a need's basis in accordance with the client's care plan.
The Home Support Worker is supervised by the Coordinator, Integrated Team Care Program (ITC) with broad direction from the Manager, Allied Health. The Home Support Worker does not have supervisory responsibilities.
This position works closely with other members of the CHSP Team and maintains regular contact with other program staff to ensure effective outcomes for clients.
Key responsibilities:
Provide direct support to clients to ensure the service is:
client-centred and providing opportunities for each client to be actively involved in addressing their goals.
- focusing on retaining or regaining each client's functional and psychosocial independence.
building on the strengths, capacity and goals of individuals.
Assist the client to attain goals as outlined in the individual client care plan to ensure their needs are being met and support the client to help them to remain living at home and an able member of the community.
Improve the client's capacity for self-management and to build confidence, by providing the client with assistance in domestic duties which may include dishwashing, house cleaning, clothes washing and ironing, help with meal preparation and demonstrating and encouraging the use of domestic assistance equipment and techniques.
Provision of transportation for CHSP registered clients to support access to services they are required or wish to attend.
Participate in staff meetings, staff development opportunities and team meetings as required.
Advise the Coordinator, ITC / Manager, Allied Health of any concerns regarding the client or support being delivered.
Complete timesheets at the end of each shift and ensure these are signed by the client.
Complete client notes for all work undertaken with clients in a timely manner.
Complete incident report forms in relation to any incidents that occur or any situations which are considered to be noteworthy or unusual.
Participate in organisational and community activities to advance the health and wellbeing of Aboriginal and Torres Strait Islander people.
Work within Wuchopperen's Policies and Procedures including the Staff Code of Conduct and the Work Health and Safety requirements and contribute to continuous improvement processes to ensure compliance with relevant standards and safe, effective service delivery.
Other duties as required, within your skills, experience and capacity.
Core Capabilities:
For applicants to be successful in this role, they should demonstrate:
Demonstrated experience working and/or providing care in the aged care sector / direct service background.
Knowledge and understanding of Aboriginal and Torres Strait Islander societies and cultures from an historical and contemporary perspective, including the ability to communicate effectively and sensitively with Aboriginal and Torres Strait Islander people.
Demonstrated ability to be flexible and responsive to the changing needs of an individual client.
Extensively developed interpersonal and communication skills including the ability liaise with a range of stakeholders as well as effectively advocate for Aboriginal and Torres Strait Islander people.
Demonstrated ability to work independently and in a multi-disciplinary team environment, prioritise and meet deadlines, deal with matters of a sensitive and confidential nature.
Demonstrated ability to meet and work within the core values and behaviours of Wuchopperen.
Qualifications and experience:
- It is a condition of employment that all employees are holders of a current and valid 'blue card' or Working with Children Check. This is at your cost. Employment cannot continue or commence if you do not have a 'blue card' or if your 'blue card' is withdrawn or is disqualified.
- Possession of a current Queensland "C" class driver's licence is essential.
Requirements:
- Hold a current Blue Card (for suitability to work with children and young people).
- Proof of qualifications is required to be provided prior to the commencement of duty.
- Possess a current driver's licence.
- A commitment to adhere to Wuchopperen's policies and procedures.
- Successful applicant to consent to a broader Criminal history check
