YNA is seeking a compassionate, experienced, and proactive Care Worker to take on a Team Leader position supporting a high-needs female client living with the effects of a Traumatic Brain Injury (TBI).
In this vital leadership role, you will be responsible for ensuring the delivery of high-quality, person-centered care while effectively managing and supporting a small, dedicated team of careers. You will serve as the primary point of contact for the client's family members, YNA office staff, and external healthcare or support stakeholders.
Key Responsibilities- Provide high-level personal and complex care, ensuring the client's safety, comfort, and dignity at all times.
- Coordinate care staff on a daily basis, including shift planning and team management.
- Liaise with medical professionals, allied health teams, and the client's family to ensure continuity and quality of care.
- Support and mentor team members to uphold consistent, person-centered care practices.
- Administer and manage enteral nutrition, including PEG feeding, in line with care plans.
- Assist with bladder management and medication administration.
- Monitor and report changes in the client's health and behaviour to relevant stakeholders.
- Ensure care plans are followed and updated as required.
- Maintain accurate and timely documentation to meet compliance and quality standards.
- Ensure care practices meet YNA's quality standards and comply with relevant guidelines and protocols.
We're looking for someone who is not only clinically competent but also a natural leader and communicator. You will have:
- Minimum 1 year experience in a complex care environment, ideally with clients who have brain injuries.
- Previous leadership or team coordination experience (highly desirable).
Proven experience in:
PEG feeding and enteral nutrition
- Bladder management
- Medication administration
- Strong communication and interpersonal skills, with the ability to use technology for documentation and communication.
- The ability to lead with empathy, professionalism, and confidence.
- A Certificate III or IV in Individual Support – Disability.
