About the Role

This position has two components of providing care coordination and physiotherapy in the community and also within the Emergency Department.

In the community, you will oversee a small caseload of clients with varied health and social needs in a person-centered approach. This role includes conducting comprehensive holistic assessments, coordinating health and social care needs, monitoring achievement of client goals, facilitating assessment and required intervention from other health care professionals to meet identified client needs, managing risk and ensuring appropriate linkages with hospital and community services. As a Physiotherapist, you will also provide assessment, intervention and management to achieve optional functional recovery, encourage self-management and promote empowerment for clients to improve their health outcomes, wellbeing and quality of life. Physiotherapy intervention is prioritised for those clients experiencing safety, function and medical issues that impact on functional independence.

ED care coordination will include comprehensive assessment of patients within our identified target groups to ensure safe discharges, referrals to appropriate services, identification of risks and ensuring allocation of hospital and community resources in an efficient and cost-effective manner. You will also provide timely and effective physiotherapy specific assessment and treatment as part of a secondary contact physiotherapy service within the ED. Common presentations include aged care and falls related injuries, musculoskeletal conditions, fracture management, vestibular, and cardiorespiratory concerns. In collaboration with your fellow ED physiotherapists and the Primary Contact Practitioner in ED (Grade 3 Physiotherapist) you will assist in the development of the ED Physiotherapy service.

Classifications for this position will be a Grade 2, Physiotherapist: VB1 – VB4 ($90, $103,724.30 per annum, pro rata)

This service aims to integrate and coordinate existing services within St Vincent's Hospital and guide the development of new services to meet the need of the target client group. Key target groups include patients with complex needs who are impacted by one or more of the following key factors: substance use, mental health issues, homelessness, disability, complex aged care issues, family violence and Aboriginal health. Operationally the role has strong links with the Health Independence Program (HIP). HIP offers a comprehensive range of specialist clinics, services and rehabilitation programs, providing rapid response health management, short-medium term care coordination, specific allied health interventions and health education. HIP enables clients to be the decision makers of their care, and support clients through self –management to achieve their healthcare goals.

4. POSITION PURPOSE
The Physiotherapist/Care Coordinator through screening and recruitment in the Emergency Department (ED) is responsible for the delivery of high quality physiotherapy intervention, as well as timely and comprehensive assessment and care coordination of patients with complex needs to facilitate the allocation of hospital and community resources in an efficient and cost-effective manner. Assessment and care planning are achieved in collaboration with patients, their carer's and other members of the Emergency Department Clinical team. In collaboration with the Primary Contact Practitioner in ED (Grade 3 Physiotherapist) assist in the development of the ED Physiotherapy service. This position can receive clinical feedback directly from the Senior Physiotherapist within ALERT and strong links with the Primary Contact Physiotherapist, which sits within the STV physiotherapy department.

5. POSITION DUTIES
 Deliver accountable, client focussed, high quality physiotherapy assessment, intervention, education and management both in the Emergency and for recruited HIP/Hfth clients in the community
 Early identification of ALERT and physio patients in the Emergency Department via screening and referrals
 Maximize a patient's functional independence and safety through assessment and treatment planning, implementation and evaluation
 Provide appropriate physiotherapy clinical management, discharge planning & community liaison for all relevant patients in both the ED 
 Identify and access multidisciplinary and specialist supports to address specific client issues.
 Collaborative care planning with clients, carers and/or families in the context of their health, functional, psychosocial, cultural, environmental and economic needs
 Facilitation of client generated goals, that are realistic, time limited and based on addressing health and wellbeing needs
 Ensuring appropriate use of brokerage funds
 Active participation in team interface processes, service development, evaluation and portfolio activities
 Perform within the HIP & HftH guidelines and specific service guidelines
 Ensuring all documentation including client records are completed and managed in a timely manner consistent with SVHM, HIP and HftH policies and procedures, legislative and regulatory requirements
 Contributing and participating in the HIP, HftH and ALERT professional development (PD) programs and other PD that supports own learning objectives for this role
 Actively participate in quality improvement initiatives (includes working groups, projects, audits, discipline related projects) as directed by Team Leader/ senior physiotherapy staff

6. INCUMBENT OBLIGATIONS

 Attend general hospital orientation within 3 months of commencement

8.1 ESSENTIAL REGISTRATION, LICENSE OR QUALIFICATION REQUIREMENTS

 Degree or Diploma in Physiotherapy
 Eligible for registration/membership with AHPRA
 Current Victorian Driver's Licence

8.2 OTHER ESSENTIAL REQUIREMENTS
 Minimum 2 years post graduate physiotherapy experience collectively in the fields of acute, subacute, or community
