At Access Care Support Services (ACSS), we're a warm, participant-focused NDIS provider committed to empowering individuals to live more independently and confidently. We deliver a wide range of disability supports across the Perth region, and we're now expanding our Support Coordination team to strengthen the quality of services we provide.

About the Role

We're looking for an experienced and passionate NDIS Support Coordinator to join our growing team.

In this role, you will work closely with participants, families, and service providers to help participants understand and implement their NDIS plans, build capacity, and connect with quality supports.

Key Responsibilities

  • Work directly with NDIS participants to coordinate and monitor supports in line with their goals.
  • Build strong relationships with families, support workers, and community networks.
  • Assist participants to navigate NDIS systems and make informed decisions.
  • Maintain accurate case notes and prepare progress reports.
  • Liaise with external stakeholders such as plan managers, LACs, and allied health professionals.

What We're Looking For

  • Previous experience in NDIS Support Coordination, Case Management, or Allied Health.
  • Strong understanding of NDIS Practice Standards and person-centred care.
  • Excellent communication, organisation, and advocacy skills.
  • Ability to work independently and manage multiple caseloads.
  • NDIS Worker Screening Check, Police Clearance & Driver's Licence.
