Community Connections Australia (CCA) is committed to promoting people's autonomy and independence.

We are seeking an experienced Support Coordinator to join our team at CCA. This position requires you to have the skills to work collaboratively with people, who have complex support requirements, to explore all the possible goals, outcomes, and opportunities when implementing or reviewing the person's NDIS plan.

Underpinning the Support Coordinator role is the willingness and capability to work with the person living with a disability to have the same opportunities, control and high expectations that most people in our communities hold for their own lives.

Key Responsibilities:

  • Work closely with people and families to understand and implement their NDIS packages
  • Support people to identify, engage and facilitate multiple service providers.
  • Educate and build capacity so as to increase their ability to manage their plans independently
  • Work collaboratively with a range of other providers/stakeholders

Selection Criteria:

  • At least one year experience as a recognised NDIS support coordinator and extensive knowledge of the workings of the NDIS.
  • At least 3 years experience working in the disability sector and recognised positive outcomes achieved working with people who may identify as having a disability.
  • Core values that have guided your work within the Disability sector.
  • Ability to work autonomously.
  • Drivers Licence and ready availability to a Car and preparedness to regularly travel in the greater Sydney area.
  • Covid Vaccinated
  • Availability: 5 days Mon to Friday.
  • Tertiary qualifications preferred but not essential.
  • Computer skills - Google
