This is a supervisory role, ideally requiring five or more years of experience supervising a multi-disciplinary team of specialists or team of clinicians.
As the Lead Clinician, you will:
- manage a team of multi-disciplinary clinicians, nursing and allied health professionals, to ensure the efficient and effective delivery of clinical advice and services to clients driven by client choice and inline with the care plan, budget, clinical governance framework and overarching service model.
- ensure clinical processes and practices are inline with the clinical governance framework through close collaboration with the broader team.
- provide support and education to clinical and allied health team members.
- provide senior expertise and advice in relation to clinical and complex care for older people in a community-based setting within an environment of increasing acuity.
- ensure that the delivery of clinical services is performed in a person-centred way that is safe and effective to meet the needs and goals of the client.
- embed a culture of reflective practice through the implementation of a clinical supervision framework.
- actively participate in the planning, implementation, and evaluation of care and clinical initiatives as required.
- analyse care and clinical data to identify opportunities for improvement in practice and outcomes, to assist with workforce development to identify gaps/opportunities for service provision.
- contribute to the development and continuous improvement of clinical practice in Home and Community Care.
- prepare and facilitate workplace education sessions and provide relevant updates, information, and support to contribute to improved person-centred care and clinical support for clients.
- review and maintain the required client documentation inline with policies and procedures.
- in collaboration with the team, identify and address future opportunities fand challenges for the services;
- maintain professional development to align with professional standards, contemporary best practice in clinical developments and organisational principles.
- travel between office, client and community sites as required.
Your experience and skills, include:
- Excellent written and verbal communication skills.
- Organised, systematic, thorough, accurate and disciplined.
- A well-developed understanding of Aged Care clinical practice and community-based service delivery.
- Demonstrated ability to apply assessment and complex case management skills.
- Demonstrated ability to work independently and collaboratively in a multi-disciplinary team and with external service providers.
- Passion for social change and advocating for the most vulnerable, at risk and disadvantaged members of our community.
- Willingness to travel.
- Minimum 5 years post-registration clinical experience in aged care or similar setting
Mandatory qualifications:
- Bachelor qualifications in Nursing.
- Current registration with AHPRA.
- Post registration qualifications related to Aged Care.
- Current First Aid certificate.
- Current and unrestricted Australian Driver's Licence.
- Own vehicle and current comprehensive car insurance.
Bonus qualifications:
- Previous experience in the Aged Care industry, preferably Home Care.
- Knowledge of the Aged Care Quality Standards.
- Post graduate qualifications in clinical practice.
