That's why we're looking for a Manager, Coordination Services to join our leadership team. This is not just a management job. It's a chance to lead with purpose, stay grounded in practice, and help shape a portfolio of programs that keep people well, out of hospital, and living the lives they want.
The Role
You will lead a team delivering coordination and care coordination services across:
- NDIS Support Coordination – helping people invest their NDIS funding in line with their goals and aspirations.
- Back to Home, Back to Life – hospital discharge support, making sure food, medications, home care, and other essentials are in place so people can recover safely.
- GP Social Work – working alongside GPs to address health equity barriers like income, housing, and access to essential services.
- Complex Mental Health Care Coordination – providing wraparound support for people living with enduring mental illness, integrating hospital, primary care, allied health, and community services.
We don't "case manage." We believe in people's skills and capabilities. This role is about supporting people to build capacity for their longer-term wellbeing, while coordinating services in the short-term to meet immediate needs. You'll also hold a small personal caseload — because leaders should stay connected to practice.

About You
You are a leader who thrives in complexity. You can balance vision with detail, and strategy with hands-on work. You are as comfortable working with a client in their home as you are collaborating with GPs, hospitals, or funders.
You bring:
- Qualifications in social work, allied health, or community services.
- Experience leading teams and services in health, disability, or social care.
- Confidence in direct practice with people experiencing complex needs.
- Strong commercial and operational acumen.
- A passion for equity, connection, and improving quality of life.
