About the Role

We are seeking an experienced Support Coordinator to join our team in the Illawarra and Greater Sydney Regions on a permanent part-time basis (3 days per week), with the potential to transition to full-time. This is a flexible hybrid role, offering a mix of working from home, meeting participants in the community, and collaborating with your team in person during regular meetings.

In this role, you will work closely with NDIS participants to strengthen their ability to coordinate and implement their plans and access supports. You will help participants build capacity and connect with the right services and providers to achieve their goals.

This role requires strong communication, problem-solving, and organisational skills, as well as a genuine commitment to person-centred practice and human rights.

You thrive on building strong relationships and working collaboratively with participants, families, and providers to deliver meaningful outcomes. Your Responsibilities Will Include

Participant Support & Planning

  • Work closely with participants to understand their goals, needs, and preferences.
  • Assist participants to implement their NDIS plans effectively.
  • Develop strategies and action plans to achieve short- and long-term outcomes.

Service Coordination

  • Connect participants with suitable providers (e.g., therapy, community, in-home support).
  • Build strong relationships with providers to ensure quality service delivery.
  • Monitor services to ensure they align with participant goals and budgets.

Capacity Building

  • Support participants to strengthen their ability to manage their own supports.
  • Educate participants on NDIS processes, budgeting, and provider choice.
  • Promote independence and empowerment wherever possible.

Monitoring & Reporting

  • Regularly review participants' plans and progress.
  • Document outcomes, challenges, and adjustments as needed.
  • Provide feedback and reports to the NDIA as required.

Problem Solving & Advocacy

  • Address barriers participants face in accessing services.
  • Advocate for participant rights and fair access to resources.
  • Escalate complex issues to the NDIA or relevant stakeholders when necessary.

Compliance & Administration

  • Ensure all support coordination activities meet NDIS guidelines.
  • Maintain accurate participant records and case notes.
  • Follow organisational policies, confidentiality, and safeguarding procedures.

Essential Requirements

  • NDIS Worker Screening Clearance (or willingness to obtain)
  • Paid Working with Children Check (or willingness to obtain)
  • NDIS Worker Orientation Module
  • Current Driver Licence

Qualifications & Experience

  • Previous experience in support coordination, case management, or a similar role.
  • Strong understanding of the NDIS framework, funding categories, and guidelines.
  • At least 2 years' experience in disability services, case management, or support coordination (Preferred).
  • Relevant qualification in disability, community services, social work, or similar (Desirable).
