Overview

Join Breakthru as a Support Coordinator in Port Macquarie, New South Wales. This role supports people to navigate the NDIS, understand and maximize their NDIS plan, engage with other services and supports, and ensure plan outcomes and individual expectations are met. You will provide individual guidance to help people become confident in managing their NDIS package and related services.

Day-to-day responsibilities
  • Understand and interpret individual plans
  • Identify service options that best meet client needs and outcomes
  • Track goals to demonstrate progress and success
  • Develop skills in clients to enable independence in managing their NDIS funding
  • Complete required documentation and administration to record progress, internal reports, and NDIS requirements
Essential criteria
  • Criminal record check (essential or willing to obtain)
  • Working with children's check (essential or willing to obtain)
  • NDIS Workers screening clearance (essential or willing to obtain)
