We are seeking a skilled and collaborative Patient Flow Coordinator to join our dynamic Access and Flow team. In this pivotal role, you'll coordinate safe, timely, and patient-centred discharges from Peter Mac inpatient wards by supporting discharge readiness from pre- admission to final handover.

What You'll Do
• Identify discharge barriers early and coordinate referrals to @Home, community, and subacute services
• Partner with ward teams, Access Coordinators, and external providers to support seamless transitions
• Lead and contribute to improvement projects targeting length of stay, discharge quality, and service integration
• Participate in flow huddles and case conferences, and escalate delays appropriately
• Use data, digital tools, and EMR workflows to support real-time discharge planning and flow decisions

What You Bring
• Registered Nurse (Division 1) or Allied Health professional with current AHPRA registration
• Minimum 5 years' experience in acute, oncology, subacute, or care coordination roles
• Proven skills in discharge planning and community/subacute referral pathways
• Strong communication, stakeholder engagement, and clinical judgement
• Proficiency in Epic, IPM, or other clinical and data platforms
• A commitment to patient-centred care, equity, and service improvement

Position SummaryThe Patient Flow Coordinator plays a critical role in supporting timely, safe, and coordinated patient discharges across Peter MacCallum Cancer Centre. Embedded within the Patient Access and Flow team, this role focuses on early identification of discharge barriers, proactive coordination of referrals to community, subacute, and @Home services, and collaborative planning with multidisciplinary teams.
Working closely with the patient and carers, ward teams, Access Coordinators, and external providers, the Patient Flow Coordinator ensures patients experience a seamless transition of care while also contributing to the hospital's strategic goals around access, length of stay, and patient-centred outcomes.
This role also supports system improvement through audit, education, and digital optimisation, aligning practice with Peter Mac's commitment to excellence in cancer care and operational efficiency.

Skills
Essential Requirements
• Registered Nurse (Division 1) or Allied Health Professional with current AHPRA registration (or eligibility for professional association membership if non-AHPRA registered discipline)
• Minimum 5 years' experience in an acute, oncology, subacute or discharge planning role
• Demonstrated understanding of access and flow principles in a complex health service environment
• Proven experience in complex discharge planning, community or subacute referral coordination
• Highly developed communication, negotiation, and stakeholder engagement skills
• Proficiency in using clinical systems (e.g. Epic, IPM) and digital tools to support patient flow
• Ability to work both autonomously and collaboratively in a multidisciplinary setting
• Strong critical thinking, time management, and documentation skills
Desirable Requirements
• Postgraduate qualifications or working towards postgraduate study in oncology, care coordination, profession specific masters or health management
• Experience working in or with @Home, HARP, or other integrated care models
• Prior involvement in quality improvement, service redesign, or digital optimisation initiatives
• Familiarity with patient flow dashboards, reporting tools, or EMR-based discharge planning workflows
• Experience in leading or delivering staff education related to access, flow, or discharge pathways

1. Patient Flow and Discharge Coordination
• Conducting complex discharge assessments using validated frameworks aligned to cancer-specific pathways.
• Proactively identifying and resolving potential barriers to discharge.
• Participating in ward-based discharge rounds and site-wide flow huddles, providing specialist clinical advice and consultation to ward teams, particularly in complex discharge planning scenarios.
• Ensuring clear, timely, and accurate documentation of discharge planning in Epic.
2. Education, Research and Quality Improvement
• Monitoring ward and hospital LOS data in collaboration with the clinical teams and identifying discharge delays.
• Leading or participating in improvement initiatives targeting avoidable days and discharge efficiency.
• Completing internal audits to evaluate discharge planning quality, barriers, and community follow-up success rates.
• Strengthening discharge planning capability through delivery of ward-based in-services, lunch-and-learns, or targeted education for new staff.
• Contributing to policy development and service redesign focused on integrated care and transition pathways.
• Supporting strategic KPI achievement related to Access and Flow.
3. Communication and Stakeholder Engagement
• Engaging in effective and respectful communication with patients, families/carers, staff, and external providers regarding discharge readiness and planning.
• Facilitating timely MDT case conferencing to support complex patient transitions.
• Representing the Access and Flow team in ward rounds, discharge huddles, and planning meetings.
• Building and sustaining strong working relationships with key community and subacute care providers.
• Navigating sensitive or delayed discharges with strong negotiation and conflict resolution skills.

• Using the EMR, iPM, and BI dashboards to support real-time decision-making and trend analysis.
• Supporting the refinement of EMR documentation workflows to improve discharge visibility and planning.

• Participating in system testing or change evaluation related to patient flow modules.
