Purpose of the Program Coordinator

To work alongside our current coordinator and amazing carers, to coordinate and lead Genovation's Young Adult Enrichment Program (YAEP) so young adults with complex support needs experience safe, meaningful, community-based days.

Transitioning from school to life after school is one of the most stressful times for young adults and their families. We aim to make this transition as enjoyable and stress free as possible.

You'll plan weekly programs, collaborate with families and the team, and ensure supports meet the NDIS Practice Standards & Quality and Safeguards.

Role Description -

Program Coordinator in the Young Adult Day Program

Location: On site at Spring Farm and in the community

Hours: Monday to Friday 7:30m to 4:30pm (most days 8:00am to 4:00pm)

Employment type: Full-time but part-time may be considered

Salary: $95,000 - $110,000 (Incl. Super) + Salary sacrifice available

Award: SCHADS or to be determined based on qualifications

The Program Coordinator will be responsible for managing the day-to-day operations of the program, developing and implementing program activities, and ensuring the program runs smoothly. Responsibilities include coordinating with participants, families, and allied health professionals, organizing events and activities, and providing excellent care to our participants. The role involves maintaining accurate participant records, monitoring participant progress, and ensuring compliance with relevant regulations and standards.

Key Responsibilities

1.Participant outcomes and program design

  • Co-design individual goals and weekly plans that build confidence, friendships and life skills
  • Knowledge of and able to implement AAC and functional communication and positive behaviour support principles
  • Plan and deliver a balanced timetable (e.g., hydrotherapy, movement/mobility, shopping & cooking, communication/social routines, creative projects, inclusive outings)
  • Ability to create and implement various communication methods (picture communication, makaton sign etc)
  • Coordinate community initiative work (e.g., packing fresh produce boxes with Box Divvy)
  • Complete and maintain risk assessments and individual support plans

2.Leadership and Supervision

  • Provide day to day direction to carers
  • Implement individual goals into function-based daily activities, includig data informed progress monitoring and staff mentoring
  • Oversee the supervision and training of new staff on the program
  • Ensure staff are working within their scope and encouraging professional progression
  • Promote a positive, inclusive culture, aligned to Genovation's core values

3.Safe
guarding, quality and compliance

  • Uphold zero-tolerance to abuse, neglect, exploitation, discrimination
  • Ensure incidents/hazards are reported and improvements implemented
  • Maintain documentation standards (progress notes, data tracking, attendance, outcome measures)
  • Ensure practice aligns to behaviour support plans and restrictive practice rules (escalating where specialised support is required)

4.Family, stakeholder and community engagement

  • Liaise and collaborate regularly with families/guardians
  • Liaise with allied health, schools, medical health professionals and community organisations
  • Represent Genovation professionally and with pride in the community

5.Administration

  • The role will include administration time, which is non face-to-face. This will include time to attend to reports, planning, collaboration, and other required documentation
  • Track and monitor individual progress and maintain appropriate records
  • Be proactive to continuous improvement for participants, staff and the organisation

Key Relationships

Participants and families/guardians, allied health, medical and educational professionals, carers, current program coordinator, team leader, care services and business development manager, chief operations officer and chief executive officer, broader community partners.

Selecton Criteria/Qualifications

  • Bachelor's degree in a related field (Special Education, Developmental Education, Allied Health) or equivalent experience
    is preferred
  • Minimum 3 years experience in the disability sector
  • Experience coordinating disability/day programs or similar community services
  • Practical knowledge of the NDIS Practice Standards, Zero tolerance framework, safeguarding, risk management and incident reporting
  • Proficient with Microsoft 365 (Outlook, teams, Excel/Forms, Word, Sharepoint)
  • Strong communication skills, especially with families, team members and various stakeholders
  • Strong written communication
  • Strong organizational and time management skills
  • Ability to work collaboratively with diverse teams and stakeholders
  • Confident leading a small team

Desirable Criteria/Qualification

  • Tertiary qualifications in Special Education, Developmental Education, Allied Heath or similar

Registration/Membership with AHPRA/CPSP/AASW or eligible.

  • Experience with hydrotherapy and accessible transport
  • Understanding of mealtime managment plans, manual handling, medication administration, peg feeding
  • Basic data collection and reporting
  • Current First aid and CPR

Inherent requirements

  • Understanding of complex care - peg feeding, epilepsy, intellectual delay, non-verbal communication
  • Able to safely perform the inherent physical tasks of the role (with reasonable adjustments), including assisting transfers, using hoists, pushing wheelchairs, standing/walking during community outings, and occasional handling of equipment.
  • Driving large modified vehicles - Mercedes Benz Sprinter (the same as an ambulance), Toyota Hiace Commuter, Kia Carnival
