The Opportunity:

We are currently seeking a skilled and passionate Care Manager to join our team in delivering exceptional aged care services.

As a key leader within the facility, the Care Manager will ensure effective management and compliance, overseeing all aspects of care delivery while promoting resident satisfaction and wellbeing. You'll be working closely with families, healthcare professionals, and management to maintain the highest standards of care.

Key Responsibilities:


• Provide quality care to consumers in line with the Australian Aged Care and Quality Standards .


• Assess, plan, deliver and evaluate care for aged consumers as part of a professional nursing and multidisciplinary team.


• Oversee the documentation of Care Planning for incoming consumers and provide leadership in the review of Care Plans.


• Assist in the supervision of staff and resource allocation as required.


• Ensure full compliance with Serious Incident Reporting Scheme.


• Promote good relationships with consumers and families by ensuring families are aware of consumers' current state and progress.


• Assist in case conferences, accurately documenting and following up on escalations and reports arising from case conferences.


• Monitor reports of consumers on accident/incidents, admission and departure


• Assist in resolving any conflicts between care staff and ensuring conflicts are handled in line with the relevant policies and procedures.


• Oversee day to day clinical activities and identifying clinical high-risk matters, escalating to the Facility Manager as appropriate.

What we're looking for:


• Current registration with the Australian Health Practitioner Regulation Authority (AHPRA).


• Demonstrated experience leading a team.


• Tertiary qualifications in Nursing.


• 5+ years' experience in a similar role.


• Demonstrated experience in improving clinical outcomes.


• Demonstrated experience working in a highly regulated environment.


• Prior experience working in an aged care setting and reporting on aged care standards and accreditation.
