As a Support Coordinator specialising in Mental Health, you will be responsible for helping NDIS participants understand, implement, and maximise their NDIS plans. This role involves coordinating a range of formal and informal supports to ensure participants achieve their personal goals and maintain stability in their mental health journey.

You will work with participants who may present with complex psychosocial needs, requiring a compassionate and strengths-based approach to coordination. The role involves collaboration with clinical teams, support workers, families, and community services to deliver holistic and effective care.

Key Responsibilities

  • Build strong, trust-based relationships with participants to understand their goals, strengths, and support needs.
  • Assist participants to navigate the NDIS and effectively implement their plans.
  • Coordinate and monitor services from multiple providers to ensure quality and consistency of care.
  • Promote recovery-oriented and capacity-building approaches to increase participant independence.
  • Liaise with mental health professionals, housing services, community supports, and family members as required.
  • Identify risks and develop appropriate safety and crisis management strategies.
  • Maintain accurate participant records, progress notes, and reporting in compliance with NDIS Practice Standards.
  • Regularly review participant outcomes and make recommendations for plan reviews when needed.

About You

  • Minimum of 2 years' experience in support coordination, case management, or mental health service delivery (NDIS experience highly regarded).
  • Strong understanding of NDIS principles, funding structures, and support categories.
  • Demonstrated ability to work with individuals experiencing psychosocial disabilities, trauma, and complex needs.
  • Excellent communication, problem-solving, and interpersonal skills.
  • Proven ability to work autonomously and manage competing priorities.
  • Current Driver's Licence, NDIS Worker Screening Check, and Working with Children Check (or willingness to obtain).

Job Type: Part-time

Pay: $38.00 – $43.00 per hour
