**About the role

Permanent part time opportunity to provide nursing care at an advanced scope of clinical practice including expanded skills and knowledge in assessment, care planning and implementation, diagnoses, and evaluation of nursing care for a cohort of consumer/patients in the specialty area of Palliative and Supportive Care for RBWH. This role works collaboratively with the interprofessional team and contributes to enhanced consumer/patient outcomes by actively leading and supporting existing and emerging nurse-led models of care whilst maintaining flexibility and responsiveness to the needs of the health service and consumer populations.

About you

Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:

Demonstrate autonomy, accountability, and competence for expanded practice in providing client-focused care within a nursing model including advanced health assessment of consumer's/patient's clinical condition, development, and implementation of appropriately documented management plan.

Evaluate service outcomes with specific indicators and benchmarking and apply appropriate change management strategies as required.

Maintain effective interpersonal and communication skills with interprofessional colleagues and extended community health providers as necessary in the course of consumer/patient management and provide specialist consultation and liaison within the scope of the position.

Comply with, and utilise procedures, policies, regulations and standards which impact upon position including contemporary human resource management requirements and practices such as workplace health and safety, equal employment opportunity and anti-discrimination policies
