Help Us Shape the Future of Aged Care
Trilogy Care is one of Australia's fastest-growing approved Home Care Package providers. Our innovative self-management model gives older Australians more choice, transparency, and value - so they can spend less on fees and more on the care they need.
We're a vibrant, purpose-driven team dedicated to helping people live independently at home for longer. It's not just about compliance - it's about making a real difference for our clients, their families, and our people. We're proud to be building a workplace where individuals feel empowered, supported, and valued.
We're growing fast and looking for a confident team leader to guide our Care team. Working under clinical supervision, the Team Leader oversees a dedicated team of Care Partners, ensuring delivery of exceptional person-centred care to care recipients. This role leads a team, provides supervision, coaching, and sets clear expectations for best practice. The Team Leader will lead by example, respecting the choices of care recipients, delivering high-quality outcomes, and upholding integrity, transparency, and ethical standards in all actions. Further, this role fosters a culture of ownership, innovation, and wellbeing, prioritising continuous improvement, the safety and empowerment of care recipients and staff, and the responsibility for delivering exceptional care.
What You'll Do
- Lead, mentor, and support a team of Care Support staff delivering high-quality, phone-based aged care services.
- Oversee day-to-day operations, ensuring consistent performance against key service metrics and quality standards.
- Track KPIs and use advanced, data-driven insights to guide team coaching, workflow optimisation, and service improvements.
- Proactively resolve client escalations and apply effective conflict management to sensitive situations.
- Facilitate regular team check-ins and development sessions to foster engagement, clarity, and professional growth.
- Collaborate closely with compliance, care coordination, and operations teams to lift overall service delivery and maintain alignment with regulatory standards.
- Continuously improve customer support processes, onboarding frameworks, and remote care management workflows.
What You'll Bring
- Tertiary qualification or Certificate in Ageing Support (highly desirable)
- Minimum 2 years' leadership experience in a human services setting (e.g. aged care, disability, NDIS, or health support)
- Demonstrated success in team development, performance management, and service delivery
- Exceptional verbal and written communication skills tailored to diverse contexts (e.g. care plans, progress notes, regulatory documentation)
- Strong interpersonal skills with the ability to mentor, coach, and engage team members effectively
- Proven problem-solving and conflict resolution capabilities
- Advanced ability to use data to inform decision-making and improve team and service outcomes
- Skilled in interpreting policy and regulatory material and applying it practically in care management
- Confident using Microsoft 365, CRMs, and digital care platforms in a fast-paced, high-volume, remote-first environment
Why Join Trilogy Care?
- Expansive new King Street office with excellent facilities, close to public transport, cafes, restaurants and supermarkets
- Vibrant team culture and regular social events
- Career mapping and progression opportunities across all departments
- Salary packaging to boost your take-home pay
