At The Only Support Coordination, we specialise in delivering high-quality NDIS (National Disability Insurance Scheme) support services to participants in regional and metropolitan Australia-wide.
Our mission is to simplify the complexities of the NDIS and empower individuals with disabilities by prioritising autonomy, independence, and participant centred outcomes. We maintain smaller caseloads to ensure we provide the highest level of personalised care and attention.
Role Description
We are seeking a Full time NDIS Support Coordinator to join our on-site team in the Essedone office based in Melbourne, VIC.
To join our dynamic team You must be passionate about delivering high-quality support under the NDIS, have strong industry knowledge, and can bring participants into the business through excellent service deliver.
You will be responsible for helping NDIS participants understand and implement their NDIS plans, connect with appropriate services, and achieve their personal goals. You will collaborate with families, allied health professionals, and mainstream services to ensure a holistic approach while maintaining a strong focus on quality over quantity.
As an NDIS Support Coordinator, you will be expected to:
- Providing exceptional Support Coordination to NDIS participants, including those with complex needs.
- Using your industry knowledge and networks to help grow our participant base.
- Meeting and exceeding KPIs by ensuring quality outcomes for participants and contributing to business growth.
- Managing risk and responding to crisis situations with confidence.
- Building strong relationships with participants, families, and stakeholders.
- What you'll need to succeed:
- Minimum 2 years' experience in Support Coordination or Case Management within the disability sector.
- Solid background in complex care, risk mitigation, and crisis management.
- Relevant qualifications in Social Work, Psychology, Mental Health, Disability, or a related field.
- Strong understanding of the NDIS, NDIA guidelines, and safeguarding participant outcomes.
- Ability to prioritise, manage multiple tasks, and work collaboratively in a fast-paced environment.
- Proven time management and organisational skills.
- Excellent communication and stakeholder engagement abilities.
- A reliable car and willingness to travel as required.

Qualifications & Experience
- A minimum of 2 years in Case Management or Support Coordination within the disability sector.
- Extensive experience in complex care and risk mitigation.
- A degree in Social Work, Psychology, Mental health, Disability Services, or a related discipline.
- Thorough understanding of the NDIS, NDIA guidelines, and how to safeguard the integrity of the scheme.
- Proven crisis management and de-escalation skills.
- Proven time management
- Ability to utilise existing network to build participants data base within the company
- Demonstrated ability to prioritise deadlines, manage multiple tasks, and work collaboratively in a team.
- Excellent interpersonal, communication, and stakeholder engagement skills.
- Bring with you an established NDIS network have proven ability to leverage an existing pool of contacts to rapidly establish effective support coordination and consistently exceed KPIs from day one.
- Much be able to travel and have a car.
- Must be able to work onsite, located in our Niddrie Melbourne Vic.
- Reliable, honest, and resourceful work ethic.
Essential Requirements
- NDIS Clearance, NDIS Worker Screening Check, Working with Children Check (WWCC), and Police Check.
- Valid Australian work rights.
- Current driver's license, access to a reliable vehicle, and proof of insurance (as per NDIS worker requirements).
