Essendon, Australia Occupational Therapist - Disability Services Full time $60,000 - $80,000 per year

Position Summary:

The NDIS Support Coordinator plays a crucial role in guiding participants through their NDIS plans, helping them access the necessary supports and services to achieve their goals. You will leverage clinical case management skills to ensure a smooth transition from hospital to home for participants and support those with mental health challenges. The role involves collaborating with multidisciplinary teams to deliver client-centered care and helping participants navigate complex systems.

Key Responsibilities:

NDIS Coordination:

  • Work with participants to understand and implement their NDIS plans, ensuring they access the appropriate services and supports.

  • Assist in identifying and connecting participants with NDIS-funded services, community supports, and clinical services.

  • Build and maintain positive relationships with participants, families, and service providers.

  • Facilitate and manage participants' transition from hospital to home, coordinating care and support to ensure safety and well-being.

  • Collaborate with hospital staff, allied health professionals, and community services to develop transition plans that meet the client's needs.

  • Monitor progress post-discharge and adjust support plans as required.

  • Support participants with mental health conditions by coordinating mental health services in alignment with their NDIS goals.

  • Use mental health frameworks to assess and respond to participants' changing needs and connect them with relevant therapeutic supports.

  • Advocate for participants to access appropriate mental health services and ensure they are integrated into their broader support network.

What we are looking for:

  • Conduct comprehensive assessments to understand participants' needs.

  • Develop, review, and monitor individualised plans, ensuring they align with clinical recommendations and NDIS goals.

  • Coordinate multidisciplinary care teams, including allied health professionals, to deliver cohesive and effective support.

  • Strong emotional awareness and ability to handle difficult conversations

  • Ability to work independently, makes decisions confidently, and demonstrates problem-solving

  • Strong Interpersonal skills, dedicated, trustworthy and reliable.

  • High proficiency in Microsoft Office and other CRM systems

Essential Criteria:

  • Relevant qualifications in Social Work, Psychology, Community Services or related fields.

  • Strong knowledge of the NDIS Act legislation and recent changes.

  • Experience in clinical, case management, hospital-to-home transitions, or mental health services.

  • Strong understanding of the NDIS framework and its application in supporting participants.

  • Excellent communication, advocacy, and interpersonal skills.

  • Ability to work independently and collaboratively with multidisciplinary teams.

  • Experience working with people with disabilities, mental health issues, or complex care needs.

  • Previous experience as an NDIS Support Coordinator or working within an NDIS-registered provider preferred.

  • Knowledge of local community services and healthcare providers.

  • Strong NDIS plan budgeting experience.

Pay: $35.00 – $40.00 per hour
