Support Partner – Empower Older Australians to Live Independently

Location: South Melbourne | Work Type: Full Time | Hybrid: 3 days in-office (Tues–Thurs)

Are you an experienced Care Manager or Case Manager who's passionate about supporting older Australians to live safely and confidently at home?

Join my client's friendly and growing Aged Care team as a Support Partner, delivering exceptional Telehealth care management and helping customers get the most from their Home Care Package.

What You'll Be Doing
  • Partner with customers and their families to develop and review personalised Care Plans and budgets
  • Guide customers to confidently manage their Home Care Package funds and make informed choices about their supports
  • Conduct Telehealth assessments, Care Plan reviews, and budget reviews to ensure funds are used effectively
  • Maintain accurate records and meet compliance and regulatory standards
  • Work collaboratively with a supportive Aged Care team that values innovation, teamwork and customer empowerment

About You
  • 12+ months of experience as a Care Manager, Case Manager, or Home Care Coordinator, ideally within a Self-Managed Home Care setting
  • Strong understanding of Aged Care, Home Care Packages, and consumer-directed care principles
  • Confident using technology and typing during Telehealth consultations
  • Excellent communication, problem-solving and documentation skills
  • Passionate about supporting customers to live independently and achieve their goals
