About us

Keep Connected Support Coordination is expanding and looking for a passionate and hard-working person to join our team This is a strictly work from home role however occasional travel to meet participants and stakeholders will be required. Keep Connected is a small independent provider located in South East Melbourne. We are a registered NDIS provider catering to all NDIS participants for Support Coordination and Specialist Support Coordination (Level 2 and 3). For any furthers questions please email

Qualifications & experience

  • Tertiary education in either Disability, Community Services, Social Work, Psychology or substantial industry experience
  • NDIS Worker Screening Clearance
  • Current Drivers License
  • Working with Children's Check
  • Previous experience working as a support coordinator or in a case management role
  • Strong knowledge of the disability and community services sector
  • Experience working with people with disabilities, child protection involvement, homelessness, Alcohol and Other Drugs and mental health
  • NDIS Online Orientation Module

Tasks & responsibilities

  • Assist clients to exercise choice and control by understanding and implementing their NDIS plans
  • Support clients to connect to NDIS and mainstream services
  • Support participants through planning meetings and reviews
  • Adhere to thorough case note recording and prepare NDIS reports as required in line with support coordination requirements
  • Meet weekly KPI targets set out by Keep Connected
  • Work autonomously with strong organizational skills and self motivation
