About the Role
As a Nurse Practitioner/ Nurse Practitioner Candidate within the CALHN BRIGHT Service, you will play a pivotal role in delivering advanced clinical assessment, rapid intervention, and comprehensive management for patients across CALHN Integrated Care sites. You will collaborate closely with a skilled multi-disciplinary team, applying your expertise to provide high-level nursing care, initiate direct clinical interventions, and coordinate care in both community and integrated care settings. This position operates with a high degree of autonomy and decision-making, allowing you to influence positive patient outcomes through innovative and evidence-based practice.
You will also contribute to hospital avoidance initiatives by maintaining up-to-date knowledge of internal and external services, supporting safe patient care outside the hospital setting, and leading clinical education and project initiatives. With responsibility for clinical leadership, direct admission pathways, and rapid assessment via the CALHN command centre, you will help shape effective service delivery. Flexibility is essential, as the role requires working across seven days and at various CALHN sites to provide expert advice and support to both hospital and community partners.
About You
To excel as a Nurse Practitioner/ Nurse Practitioner Candidate, you will bring a commitment to clinical excellence and advanced patient care. You are a highly motivated nursing professional with demonstrated leadership, communication, and problem-solving skills, currently undertaking a Master of Nurse Practitioner and eligible for registration with the Nursing and Midwifery Board of Australia or endorsed for registration with the Nursing and Midwifery Board of Australia
- Proven ability to work autonomously and collaboratively, fostering a supportive team environment.
- Strong leadership, communication, and change management skills, with a focus on consumer engagement.
- Extensive post-registration nursing experience, advanced clinical assessment, and out-of-hospital care expertise.
- In-depth knowledge of professional standards, legislation, and quality and safety practices in nursing.
