Primary objective

As a member of the multidisciplinary health team coordinate and provide culturally responsive and effective holistic health care

service for clients with multiple needs and specialities; including treatment of illness and the promotion of health and well-being

to achieve better health outcomes and prevent repeated hospitalisations.

Context statement

This position is based in the Lorraine Brennan Centre and requires an experienced and proficient practitioner who is able to operate

at an independent level against a specific set of rules and/or procedures set by the health service provider; is flexible and has the

ability to respond to complex situations and clients with multiple health conditions and within the scope of practice and health care

plans.

Key duties and responsibilities

1. Provide culturally responsive health care practice including supporting clients and their families in developing and managing

their health care plan as co-decision makers as required and within a defined scope of practice.

2. Support Aboriginal people to navigate the health system, ensuring continuity of care across the healthcare continuum

  1. Provide cultural support to other health professionals to build cultural competence and responsiveness.
  2. Participate in the organisation's Continuous Quality Improvement (CQI) process for the provision of safe, effective and efficient

health care; including enhancing clinical skills and knowledge relating to the provision of health care in subacute clinical setting

of Lorraine Brennan Centre.

  1. Provide clinical supervision, mentoring, and support to trainee and new graduate Aboriginal Health Practitioners to develop

their skills, knowledge and professional capabilities.

  1. Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken

in order to ensure high quality, safe services and workplaces.

  1. Foster a positive workplace culture through exemplary practice and role modelling behaviour.

Selection criteria

Essential

1. Current registration with Ahpra as an Aboriginal and/or Torres Strait Islander Health Practitioner.

2. Minimum AQF level 5 as per NTPS Aboriginal Health Practitioner Enterprise Agreement.

3. Proven experience providing Aboriginal and Torres Strait Islander Primary Health Care practice to individuals and families as an

advanced and proficient practitioner; including care coordination and health education and promotion.

4. Demonstrated experience in providing Aboriginal and Torres Strait Islander Health Care practice to individuals and families

within the health care system; providing effective communication ensuring clients understand information received and are

providing informed consent.

  1. Demonstrated local cultural knowledge, respect and safety at the individual and families levels and ensuring and maintaining

appropriate confidentiality.

  1. Working knowledge of health information systems used in the clinical setting; patient care systems, with attention to detail in

recording data.

and participating in a learning agreement to obtain the required qualification.
