Primary objective

Lead and deliver the care-coordination pilot programme within the RDPH Division of Emergency Medicine.

Context statement

This position, based within the RDPH Division of Emergency Medicine, is responsible for managing a pilot programme aiming to enhance

care co-ordination for frequent attendees, with a focus on reducing fragmented care by streamlining access across the health-social care

continuum. Aboriginal Health Practitioners at this level are Senior Aboriginal Health Practitioners and have a significant degree of

autonomy and decision making within their scope of clinical practice. Working collaboratively within a multidisciplinary team, your focus

will be engaging and enrolling patients through informed consent, provide ongoing case management support through advocacy and

coaching with the goal of transitioning to long term community-based care. Training and development opportunities will be supported and

negotiated.

Key duties and responsibilities

1. Provide leadership to the multidisciplinary health care team through providing a specialised high level of health care and education to

patients with complex needs to build both cultural and clinical competence.

2. Provide timely and culturally safe care, advice and health education to a patient group with diverse and complex care requirements

3. Work within a multidisciplinary framework and provide feedback to the senior staff and management on program and clinical issues.

4. Prioritise and manage a caseload within a changing work environment to ensure timelines and health outcomes are met.

5. Engage with community members and key stakeholder organisations to strengthen community involvement in health service delivery.

6. Follow defined service quality standards, work health and safety policies and procedures relating to the work being undertaken in

order to ensure high quality, safe services and workplaces.

7. Foster a positive workplace culture through exemplary work practices and role modelling behaviour.

Selection criteria

Essential

1. Current registration with Ahpra as an Aboriginal and/or Torres Strait Islander Health Practitioner with no conditions.

  1. Minimum AQF Level 6 -as per NTPS Aboriginal Health Practitioner Enterprise Agreement.

  2. Demonstrated experience providing Aboriginal and Torres Strait Islander primary care practice as an experienced and advanced

practitioner.

4. Proven organisational skills and ability to coordinate and manage programs.

5. Well-developed communication, interpersonal skills, negotiation and conflict management skills, both verbal and in writing, to

maximise productive working relationships with internal and external stakeholders.

6. Demonstrated local cultural knowledge, respect and safety at the family and community level.

7. Must be able to provide confirmation of Aboriginality; obtain a criminal history check and clearance; and working with children's

clearance (Ochre card).

Further information

period of two years, subject to engaging and participating in a learning agreement to obtain the required qualification. The successful

notice and criminal history checks. A criminal history will not exclude an applicant from this position unless it is a relevant criminal history.
