Role Description
We are seeking a passionate and motivated
Support Coordinator
to join our team in Cobblebank, VIC.
This role will commence on a casual basis while we build your participant caseload together. During this period, you'll receive hands-on training, work with a small caseload, and gain confidence in the role. Once established, you'll have the opportunity to transition into a full-time role with ongoing professional development and career growth opportunities.
As a Support Coordinator, you will work directly with NDIS participants, their families, and other stakeholders to ensure participants receive the right supports to achieve their goals.
Your responsibilities will include:
- Assisting participants to understand and implement their NDIS plans
- Coordinating and overseeing supports to ensure services meet client needs and goals
- Building strong relationships with participants, families, and service providers
- Maintaining accurate records and case notes
- Supporting participants to build capacity and increase independence
- Ensuring compliance with NDIS standards and organisational policies
- Monthly budgeting and monitoring to ensure that NDIS plans are utilised correctly
- Preparing NDIS compliance reports and documentation
Qualifications & Skills
- Friendly, approachable, and positive attitude to contribute to our amazing team
- Experience in case management or support coordination (preferred but not mandatory for the right applicant)
- Excellent organisational and problem-solving abilities
- Proficiency in case management software and digital tools
- Knowledge of the NDIS framework and person-centred practice (preferred)
- Ability to work collaboratively within a team and independently manage caseloads
- Relevant qualifications in Social Work, Psychology, Community Services, or related field (desirable)
- Current driver's licence and reliable vehicle (mandatory)
- Strong computer and technology skills (Outlook, Word, OneDrive, etc.)
- Strong interpersonal and communication skills
- Adaptability as the NDIS is always changing
