Location: _**Victoria Park WA**_

Days: Monday to Friday

Timings: 8:30am to 5:30pm
- **Hourly Rate: $50 level 2**_

**Responsibilities**:

- Coordinate the delivery of support services to clients, including developing individualized plans, identifying and arranging services, and monitoring progress.
- Work collaboratively with clients, families, and other stakeholders to understand their needs and preferences.
- Conduct assessments and develop plans to support clients' goals and outcomes.
- Identify and address gaps in services and provide solutions to meet clients' needs.
- Maintain accurate records of client interactions, assessments, and service delivery.
- Collaborate with other team members to ensure effective communication and coordination of services.
- Stay up-to-date with relevant laws, regulations, and best practices in the field

**Requirements**:

- Bachelor's degree in Social Work, Psychology, or a related field.
- Experience as a Support Coordinator
- Fluency in English (written and spoken).
- Strong communication and interpersonal skills.
- Ability to work effectively with diverse populations, including individuals with disabilities and special needs.
- Excellent organizational and time management skills.
- Ability to work independently and as part of a team.
- Strong problem-solving skills and ability to think critically.
- Familiarity with relevant laws, regulations, and best practices in the field.
