About the Role
We are seeking an experienced and passionate Support Coordinator to join our dedicated team. In this role, you will work closely with NDIS participants to help them understand, implement, and maximise their NDIS plans. You will coordinate supports across multiple providers, build participants' capacity for independence, and ensure compliance with NDIS Quality and Safeguards standards.
Key Responsibilities
- Assist participants to understand and use their NDIS plans effectively.
- Build connections with service providers, mainstream services, and community supports.
- Coordinate services to ensure participants receive responsive, effective support.
- Provide crisis support and risk management where required.
- Maintain accurate case notes, reporting, and compliance documentation.
- Support participants to build skills in self-advocacy, budgeting, and decision-making.
What We're Looking For
- Experience in support coordination, case management, or the disability/community services sector.
- Strong knowledge of the NDIS and person-centred practice.
- Excellent communication and problem-solving skills.
- Ability to manage a caseload and work both independently and as part of a team.
- Relevant qualification in Social Work, Community Services, Disability, or related field (preferred).
- Current driver's licence, NDIS Worker Screening Check, and Working with Children Check.
- Knowledge of NDIS legislation and operational guidelines.
- Experience using client management systems Shiftcare.
- Strong stakeholder engagement and advocacy skills.
