**Looking for a experienced Support Coordinator to coordinate the delivery of support services to our clients.**

Location: _**Victoria Park WA**_

Days: Monday to Friday

Timings: 8:30am to 5:30pm

Hourly Rate: _**$50 level 2**_

**Responsibilities**:
Coordinate the delivery of support services to clients, including developing individualized plans, identifying and arranging services, and monitoring progress.

Work collaboratively with clients, families, and other stakeholders to understand their needs and preferences.

Conduct assessments and develop plans to support clients' goals and outcomes.

Identify and address gaps in services and provide solutions to meet clients' needs.

Maintain accurate records of client interactions, assessments, and service delivery.

Collaborate with other team members to ensure effective communication and coordination of services.

Stay up-to-date with relevant laws, regulations, and best practices in the field.

**Requirements**:

- Bachelor's degree in Social Work, Psychology, or a related field.
- Experience as a Support Coordinator
- Fluency in English (written and spoken).
- Strong communication and interpersonal skills.
- Ability to work effectively with diverse populations, including individuals with disabilities and special needs.
- Excellent organizational and time management skills.
- Ability to work independently and as part of a team.
- Strong problem-solving skills and ability to think critically.
- Familiarity with relevant laws, regulations, and best practices in the field.
