**About the role**
We are currently recruiting for someone to join our team as a Support Coordinator and Intake role. As Intake the role will see you working alongside our General Manager, meeting new participants and completing the intake processes to ensure a smooth transition for participants to commence their requested services with us. As our support coordinator you will be liaising with our participants and assisting them to get the most out of their NDIS funding. Manage a smaller caseload of NDIS clients, supporting them to identify, coordinate and implement a range of supports to enable greater individual participation in their home and their community.
Our office is based in Carrum Downs VIC. This support coordinator and intake role directly contributes to the health and wellbeing of our participants through providing high quality access to in-home care and community environments, along with ensuring that our policies and procedures are followed.
**Duties and responsibilities may include, but is not limited to;**
Engage with participants, families and all other stakeholders involved in our company Administration of NDIS plans, implementation of plans and reporting
- Accurate and timely data entry pertaining to NDIS
- Provide timely and comprehensive information and advice in response to enquiries from potential and current clients / families regarding NDIS
- Manage a caseload of Support Coordination clients and provide appropriate support to link participants and families to services and supports within their community
- Meet with participant and families to discuss how they will access NDIS services during the NDIS plan implementation process
- Develop, continuously improve and streamline our internal processes to ensure efficiency, scalability and sustainability
- Ensure that databases and systems are kept up to date
- Scanning, filing and collating documents where required
- Take part in training sessions
Provide assistance where required to ensure the company runs efficiently Reception duties including;
- directing telephone callers to appropriate staff;
- issuing and receiving standard forms;
- maintaining basic records;
- Develop and maintain relationships within the Melbourne community and referral network
- Respond to enquiries from potential referrals, new participants and their families
- Maintain current and accurate client records using the MAAC Care software
- Liaise with internal and external partners to manage the requirements of Intake
- Ensure yourself and others in the office follows health and safety regulations
- Ensure all OH&S compliance is strictly adhered to
- Attend new business meetings with other staff members when requested
- Experience in NDIS Support Coordination, Planning and Disability services, coupled with administrative experience
A willingness to learn, to take ownership of work, use Initiative and be resourceful Knowledgeable customer service experience or demonstrate similar attributes
- Attention to detail as well as effective time management, organisation and problem-solving skills
- Proficient written and verbal communication skills
- Proficient use and knowledge of the Google Suite programs
- Proven ability to use interpersonal skills in a team environment. Work collaboratively with a range of stakeholders including peers, employees, external organisations
Qualifications in Human, Community and/or Disability Services is highly desirable National Police Check & NDIS Worker Screening Check
- Working with Children's Check
- NDIS Workers Orientation Certificate
- Valid Australian Driver's Licence & own vehicle
- Excellent communication and interpersonal skills
- Up to date COVID 19 Vaccinations
**Job Type**: Part-time (15 - 30 hours per week)
Pay Rate: $30-45 an hour plus super
