The Role
The role of the Clinical Care Coordinator is to provide clinical assessment to all Home Care Packages clients and clinical leadership to the Care Management, Service Coordination and Leadership teams. This will ensure the continued success of the home care programs at and the overall coordination of the team. The Clinical Care Coordinator is a registered nurse. The Manager In Home Support, provides operational oversight and the General Manager Quality Risk and Compliance provides clinical oversight.

Client comprehensive Clinical Assessment
- Conduct comprehensive clinical assessment for clients using standardised tools, within program specific requirements.
- Provide clinical leadership, oversight and advice to management team and care management teams of client clinical needs.
- Formally review client care plans to ensure compliance to standards of care for quality review.
- Monitor, evaluate and review care plans as required by client care needs and alert Care Management of any changes in the client's health.
- Record and monitor significant health conditions.
- Meet assess rather than meet? clients' clinical care needs under wound care, behaviour, pain and falls and recorded in clinical needs tool.
- Liaise with and refer to allied health, dementia and continence services and other medical practitioners as required.
- Escalate any clinical concerns/referrals to the appropriate person or department for review
- Ensure follow-up is in place within 1-2 business days of receiving enquiries or internal referrals.
- Provide support and oversight to care managers for clients that have been admitted to hospital and participate in discharge plan meetings
- Review new client referrals and identify clinical risks associated with client's health conditions,
- Lead High Risk client meetings and support care advisors with any clinical issues
What We're Looking For
Qualifications
- Registered Nursing with current AHPRA registration.
- Current First Aid and CPR certification.
- Valid driver's licence and access to a reliable vehicle.
Experience
- Demonstrated experience in clinical assessment and care planning for older people, preferably within a home care or community services setting.
- Demonstrated experience working in community-based nursing programs
- Experience providing clinical leadership, mentoring staff, and supporting continuous improvement in care delivery.
- Previous experience and understanding of home care teams.
- An understanding of the range of community funding programs (e.g. HCP, CHSP and DVA).
- Proven ability to manage complex clinical client cases and coordinate multidisciplinary services.
Essential skills
- Strong clinical assessment and problem-solving skills using standardised tools.
- Excellent written and verbal communication skills, including documentation and report writing.
- Proficiency in maintaining compliance with Aged Care Quality Standards and regulatory requirements.
- Effective time management and organisational skills with the ability to prioritise competing demands.
- High level of computer literacy and ability to use client management systems and digital health records.
- High level problem solving and analytical skills.
- Excellent verbal and written communication and interpersonal skills.
- High level clinical skills.
- Clinical specialty areas in wound management, continence, medication management.
- A commitment to quality outcomes for clients, employees, and volunteers to multiple tasks.
- The ability to work in a culturally sensitive manner within an ethno-specific environment.
- A preparedness to work to the 10 principles of the Eden Alternative philosophy.
- Self-motivated with a flexible, caring and patient attitude.
