I-HDS is seeking an experienced Clinical Care Coordinator (Registered Nurse) to join our team. This role combines clinical leadership and accommodation management, ensuring that our Supported Independent Living (SIL) homes are compliant, well-run, and deliver high-quality, person-centred support to participants.

**About the Role**

As the Clinical Care Coordinator, you’ll oversee the **clinical care and operational compliance** of SIL homes within your portfolio. You will:

- Ensure SIL homes operate safely, effectively, and in line with NDIS and regulatory standards.
- Provide clinical guidance, oversight, and support to staff and participants.
- Conduct participant assessments, develop care and support plans, and monitor outcomes.
- Supervise and mentor Support Workers, promoting best practice in clinical and accommodation settings.
- Build and maintain strong relationships with participants, families, support coordinators, and external providers.
- Maintain accurate client documentation, records, and compliance reports.
- Attend care team meetings and ensure consistent communication across all stakeholders.
- Contribute to a positive, collaborative culture that values professionalism, compassion, and accountability.

**About You**

You are a **Registered Nurse (AHPRA Registered)** with a genuine passion for empowering participants to live safely and independently. You bring strong leadership, coordination, and problem-solving skills, along with experience in NDIS or community care settings.

You will also have:

- Minimum 1 year of experience in a clinical, coordination, or leadership role
- Experience in Supported Independent Living (SIL) or disability services (preferred)
- Strong understanding of compliance, quality, and NDIS standards
- Excellent communication and documentation skills
- Current driver’s licence
