Are you a passionate, active individual looking for the opportunity to make a real difference in the lives of people with a disability? As an NDIS Support Coordinator you will assist participants to understand and implement their NDIS plan by linking the participant with services that meet their individual needs and monitoring their NDIS plan outcomes and expenditure. Support Coordinators are the face of Darebin Support Services. As such in this role you will be developing professional relationships with NDIS participants, families and other service providers.
- **Position Summary**_

The Support Coordinator/Case Manager's primary responsibility is to provide varying levels of coordination for National Disability Insurance Scheme (NDIS) participants, Transport Accident Commission (TAC) participants and other participants from other funding bodies. Your focus will be to help participants strengthen their ability to coordinate their supports, and support participants to choose and control the support they need to live an ordinary life.

The Support Coordinator/Case Manager will adopt a person-centred practice, which facilitates choice and control so that participants can achieve their goals and outcomes identified in their approved plan in a safe environment. Working within the NDIS framework where applicable, you will facilitate and coordinate the implementation of supports in participant’s plan including informal, funded, mainstream and community supports ensuring that all services are directed by the individual’s needs and goals.

- **Required**_
- NDIS Worker Orientation Module Certificate
- Current NDIS Worker Screening Clearance
- Current First Aid/ CPR
- Working with Children Check
- Diploma Community Services or equivalent experience
- Experience in the disability sector
- Demonstrated high level of verbal communication and interpersonal skills
- Positive, empathetic and willing to learn new skills
- Adaptable to an ever-changing disability landscape
- Microsoft suite knowledge, with a proficiency in Word and Outlook
- Time Management and Autonomous working skills
- Drivers license

- Experience as a disability case manager/NDIS Support Coordinator
- Understanding of the NDIS and its operational guidelines and framework
- Detailed report writing skills

60-75K Pro rate

**Job Types**: Full-time, Part-time
Part-time hours: 18 per week

**Salary**: $60,000.00 - $75,000.00 per year

Schedule:

- Flexible hours
- Rotating roster

Ability to commute/relocate:

- Preston, VIC: Reliably commute or planning to relocate before starting work (required)

Licence/Certification:

- Working with Children Check (preferred)
- Driver Licence (preferred)

Work Authorisation:

- Australia (preferred)
