Acura is looking for a passionate, experienced Service Coordinator to join our client in Western Sydney. You'll be joining a leading Aged Care provider, with a strong presence in NSW, QLD & VIC, who are authentically passionate about making a genuine impactful difference to help ensure clients are able to live as independently in their own homes for as long as possible.

About your new role:
You'll play an important part in maximising some of our most vulnerable community members' ability to remain living independently in their own homes, through effective coordination of tailored services with the utilisation of available funding.

Additionally, you will be required to assist with supporting the rostering coordination team to ensure the right care workers are matched with the right client either internally or through their brokerage partners.

About you:

- Minimum 1 years experience in a similar role
- Ability to provide positive solutions and outcomes to complex problems
- Strong understanding of either HCP, CHSP, TCP, or NDIS funding packages
- Ability to work effectively as a team and also independently
- Recent national police checks or willingness to obtain one
- Relevant tertiary qualifications in Aged Care or Disability
