**Position Description - Intake & Relationships Manager**

The Intake & Relationships Manager plays a key role in assisting us to identify, facilitate, and
support the necessary processes for National Disability Insurance Scheme (NDIS) participants who
may be eligible for Specialist Disability Accommodation (SDA) to achieve the opportunity to
transition into their new home.

**Responsibilities include**:
Managing Leads and - Be the central point of contact and manage the participant’s journey
Assessment from enquiry, the SDA qualification/approval process, right through
- to transition into final SDA home.- Manage initial assessment, intake, and onboarding of new
- participants into their new homes.- Track the intake process through our CRM platform Hubspot.
- Clear and accurate record keeping of information.

Resident Relations - Collaborate with NDIS participants, their families, Support
Coordinators, Occupational Therapists, and Allied Health
Professionals to make sure that accommodation options are suitable
for the participant.
- Regular site visits and tours with potential participants to show them

through their future home.
- Review of participant plans and profiles.

Fostering a positive - Foster and maintain strong relationships with Support Coordinators,
referral network Occupational Therapists, and Allied Health Professionals that result
- in positive referrals of participants.- Ensure high-quality performance of our Support Provider / Supported

Independent Living (SIL) partners who play a key part in ensuring our
participants make our apartments their long-term home.

Be a MyLife Housing - Attend Expos and other events to proactively promote MyLife
Ambassador Housing.

**Capabilities and Experience**:
With a desire to work collaboratively in a small dynamic organisation, you are autonomous, pro
- active, with a can-do attitude and nimble even under pressure to achieve results. Your areas of
knowledge and expertise that matter for this role:

- Ability to foster positive relationships with a range of people of diverse backgrounds.
- Demonstrated understanding of the NDIS, preferably in the context of SDA.
- Experience with reading and understanding Allied Health Reports, including Occupational
- Therapy reports.- Comfortable navigating your way through complex environments, ideally in the area of
- National Disability Insurance Agency (NDIA) funding and the NDIS scheme.- A working knowledge of CRM systems and Microsoft Office suite.
- A current NSW Drivers Licence and a willingness to travel to the MyLife Housing SDA sites
- currently situated across Sydney and the Illawarra region.- Acquire and ensure all relevant NDIS screening checks, including Working with Children,

NDIS Workers Screening Clearance or Police checks are maintained and kept up to date.

Reporting Line Chief Executive Officer

Location The location of work is flexible and will require time spent at the office
and at MyLife Housing SDA sites.
