**About the Role**:
Worthy Ability is seeking a compassionate and dedicated **Key Worker** to join our growing team. In this role, you will play a vital part in supporting individuals with disabilities to achieve their goals and enhance their quality of life through person-centred and strengths-based care.
You will work closely with clients, their families, and a multidisciplinary team to ensure high-quality services are delivered in alignment with the NDIS framework.
**Key Responsibilities**:
- **Client Support & Advocacy**: Serve as the primary point of contact for clients, offering guidance and advocating for their individual needs.
- **Care Coordination**: Develop and implement tailored care plans in line with each client’s goals and their NDIS funding.
- **Family & Stakeholder Engagement**: Collaborate with families, carers, and allied health professionals to deliver holistic and consistent support.
- **Progress Monitoring & Reporting**: Track client outcomes, revise support plans as needed, and maintain accurate documentation according to NDIS requirements.
- **Training & Support**: Equip families, carers, and staff with the necessary tools and strategies to support behavioural and developmental progress.
**Essential Skills & Qualifications**:
- **Qualifications**: Bachelor’s degree in Early Childhood or a related field.
- **Experience**: Prior experience working with individuals with disabilities, preferably within the NDIS framework.
- **Communication**: Excellent interpersonal and written communication skills to effectively engage with diverse stakeholders.
- **NDIS Knowledge**: Solid understanding of NDIS policies, funding structures, and person-centred approaches.
- **Problem Solving**: Ability to think creatively and develop tailored solutions for clients.
- **Teamwork**: Willingness to collaborate with a multidisciplinary team and share knowledge for collective success.
