Our Integration team is growing and we have an opportunity for an experienced, innovative and passionate primary health care professional to join our team as a Capacity Building Coordinator, specialising in Population Health.
The position is offered as either full-time or part-time and will have the flexibility to work from home 1 day per week post induction and training.

**The Role**
The Capacity Building Coordinator - Population Health will work closely with Adelaide PHN commissioned service providers, key partners and stakeholders to ensure the implementation and delivery of commissioned services and build on their existing capabilities.
This specialised Capacity Building Coordinator role has a **key focus on commissioned service providers delivering services across our priority areas including early intervention, population health, and workforce.**
**Our Capacity Building Coordinators**:
- work with existing and newly commissioned service providers with a focus on supporting clinical and non-clinical service providers to build and sustain capabilities across the primary care sector;
- establish and maintain effective partnerships with commissioned service providers;
- are responsible for the management of contracts with commissioned service providers;
- facilitate connections and integration between commissioned service providers to influence best practice service delivery;
- foster a quality improvement approach to evaluation, assessment, monitoring and redesign; and
- participate in broader systems level partnerships and integration.
**About You**
To be successful in this role you’ll need:
- Tertiary qualifications in health or human services or management (or other relevant discipline); or at a minimum, experience at a senior level within a health organisation.
- Significant demonstrated experience in building and influencing beneficial partnerships and professional relationships with a wide range of stakeholders, including health providers / peak bodies, community services, government departments, employers and the general public.
- Demonstrated experience in contract management (from either a service provider or funding body perspective).
- Experience in using standard project management techniques and knowledge, or the ability to acquire knowledge of basic scoping methodologies.
- Expertise and experience in critical analysis of complex issues in service delivery and service development and knowledge of the principles of commissioning from a primary health care perspective.
- Demonstrated experience and expertise in the review and analysis of program level budgets.
- Understanding of, and ability to practice in a manner consistent with, the principles of Primary Health Care and knowledge of the Australian primary health care sector.
- Working knowledge of capacity building principles.
- Knowledge of population health principles.
- You may have had experience in previous roles in the primary health sector such as: Contract Manager, Contracts Administrator; Senior Project Officer; Capacity Builder; Clinical Coordinator; Business Development Service Coordinator; Primary Health Care Officer; Business Development; Team Leader / Manager; Project Manager; Data Analysis and Evaluation; Policy Officer; Program Manager; Primary Health Care Manager; Health Services Manager; Clinical Manager; Service Manager; Partnerships Manager; Business Manager; Senior Manager Health; Health Director; Health Officer._
