Specialist Support Coordinator

**Introduction**:
**Your role responsibilities will include**:
- Interpret and implement participant NDIS Plans while working within the context of the NDIS framework, mainstream and community services.
- Operate collaboratively with stakeholders, providers and multi-disciplinary teams in the coordination of a participant's NDIS funded supports, mainstream and informal supports for a sustainable approach across all areas.
- Identify suitable supports for and engage with new and existing support providers while working towards achieving participant's identified outcomes and goals.
- Holistically work on points of crisis, barriers to engagement and participant risks and seek solutions to minimise ongoing risks.
- Build and sustain strong collaborative relationships with service providers, allied health professionals and government and non-government bodies.
- Continually working to build participant’s capacity with a long-term goal to self-direct, self-advocate, have direct engagement with their supports and manage their NDIS plan and their life the way they choose.
- Provide thorough case-note recording, compile and prepare reports for management and other external stakeholders in line with the NDIA Support Coordination/Specialist Support Coordination requirements.
- Ensure achievable KPI's are reached and maintained while providing person centred services.
- Provide professional direction and support in delivery of care and best practice to clients while ensuring compliance and legislation is always adhered to.
**Description**:
**What you need to be considered for this position**:
- Allied Health or other tertiary qualifications in the industry.
- Demonstrated experience working collaboratively within a team, with excellent communication skills and a willingness to share and for continuous learning.
- Excellent relational skills that value the individual and cultural diversity, fostering productive working relationships with your team and with a wide range of stakeholders.
- Preferred experience working in Support Coordination, Case Management or similar role, identifying and connecting participants to services that cover a diverse range of support needs.
- Have a demonstrated ability to identify and provide a range of suitable solutions to complex challenges, barriers and risks that face participants and work with stakeholders to manage such challenges.
- Can think outside of the box and display creative thinking to find solutions.
- Have in-depth working knowledge of the National Disability Insurance Scheme (NDIS), rules and operational guidelines. Experience in coordinating a range of therapeutic and community-based supports and methods to achieve participant goal.
- Experience in the industry with a sound understanding of NDIS legislation and compliance
- Working knowledge/skills / experience in complex mental health, complex health conditions, Housing, and justice is desirable

- Have a current open driver’s license
- Hold a current or are able to obtain DCSI Working with Children
- Hold a current or are able to obtain a National NDIS Worker Screening Clearance or DCSI Disability
- Hold a current or able to obtain a National Police Check
- Australian Citizenship or Permanent Residency
- Evidence of receipt of all applicable Covid Vaccinations
