Specialist Support Coordinator Job Description

This is an exciting opportunity to work as a Specialist Support Coordinator, providing support and coordination services to individuals with complex needs.

  • Work alongside participants to build capacity, navigate the NDIS system, and achieve their personal goals.
  • Be the key connection point between participants, service providers, and support systems, ensuring plans are implemented effectively and barriers are addressed with creativity and empathy.

Requirements:

  • Experience working as a Specialist Support Coordinator and/or in the community and disability services sector.
  • Relevant tertiary qualifications within allied health, nursing, social work, disability or mental health.
  • Driver's license and reliable vehicle.
  • Current clearances required or willingness to obtain Working with children Check, NDIS worker screen Check, police clearance and child safe environment training.
  • Experience in supporting individuals with complex or high-level needs.
  • In-depth understanding of the NDIS framework and service environment.
  • Ability to confidently liaise with stakeholders and write concise professional reports.
  • Strong organisational and interpersonal communication skills.
