**About the role - Part-time, Contract**
The **Support Coordinator** is part of a specialised team providing allied health and support coordination services to people living with a disability. The Support Coordinator will have the skills, qualifications, and experience to assist NDIS participants to connect with supports, build capacity and resilience and negotiate solutions with multiple stakeholders to achieve a well-coordinated plan implementation in line with their goals. The role will also support participants overcome barriers impacting their ability to implement their plan and their informal support networks.
- Reporting to the Manager, Specialist Support Services, duties include but are not limited to: _
- Support people with disability to connect to NDIS, mainstream and community supports and negotiate solutions with multiple stakeholders to achieve a well-co-ordinated plan implementation
- Develop a comprehensive understanding of client’s needs, preferences, and their plan, promoting individual choice and control in the selection of appropriate services and providers
- Develop and maintain effective and collaborative professional relationships with key stakeholders, both internal and external, meeting the needs of the client, including clients, families, guardians, community organisations, government agencies and the NDIS
- Lead a person-centred approach to client engagement and service delivery and ensure all clients, their families and advocates are treated with dignity and respect
- Work with other team members to improve service outcomes for clients and employees through effective complaint management, stakeholder feedback and responses to service audit processes
**About you**:
The successful applicant will have previously worked with vulnerable and at-risk clients with disability and complex needs. You will have a comprehensive background working within the NDIS framework, business and regulatory environment, rules, practice standards and Code of Conduct. You will have strong interpersonal skills and be able to build rapport quickly between yourself, clients, and stakeholders.
- You will also have the following skills and experience: _
- Tertiary qualification in a relevant field; human services, disability, community services, psychology, nursing, etc.
- Excellent verbal and written communication skills, including an ability to provide effective and appropriate advice and information to a variety of people
- Ability to maintain discretion with confidential information and the ability to manage crisis situations
