We are working with a respected aged care services organisation, who are looking for a Healthcare Case manager in Melbourne in an office
- based full-time permanent opportunity

In this newly created role, you’ll use your case management, customer service, and passion or experience to truly make an impact on your clients and their needs.

A typical day as a Case Manager includes:

- Coordinate resident clinical care needs via telehealth with clients and family
- Work in collaboration with and consistently communicate with clients and their families and manage to facilitate and develop service offerings.
- Work closely with multidisciplinary teams to provide clinical updates on clients
- Manage and monitor consumers budgets and ensure that they comply with organisation requirements and funding guidelines.
- Database management
- Ad hoc administration

You will be part of a multi-disciplinary team that works collaboratively with ongoing support from management.

**About you**

Building relationships and trust amongst your customer, you will have a genuine interest and passion towards aged care and putting the ageing Australians and their families at the heart of what you do.

**You will**
- Experience working in Healthcare, aged care or community services
- Hold a current Police Clearance (or be willing to obtain)
- Strong Administration, CRM and MS Office Skills
- Current First Aid Certificate and current CPR certificate
- The right to work in Australia
- A current driver license, registration, and access to your own car with comprehensive car insurance
