**About the role**
As a Case Manager, you will play a pivotal role in ensuring clients care needs & supports as set out in their care plans are delivered in a timely & professional manner. You will provide high quality case management services for clients, directly supporting & providing patient diagnosis, treatment, testing, and ongoing care and making a real difference to clients lives, allowing them to remain living independently within their homes & communities.
You will also have to maintain accurate and up to date case notes and be able to respond to client feedback and manage client issues and outcomes.
**Key selection criteria**
- Relevant tertiary qualification in Social Work, Allied Health, Nursing, Case Management, or equivalent
- Demonstrated experience managing accounts & clients within healthcare, banking or the corporate sector
- Knowledge of Aged Care/NDIS services & standards - ideal
- Excellent verbal and written communication and interpersonal skills
- Excellent IT & organisational skills
