Job Title: Client Coordinator

Reports to: Service Coordinator

**Job Description**:
Always About Me is currently experiencing growth and we are seeking experienced individuals to step into our Client Coordination Department & Service Delivery. As a part of this team you will be responsible for engaging with internal, external stakeholders, from the first contact query at referral to service delivery and to reviewing the service delivery from day 1 to ongoing with the clients you meet and form relationships with thereafter.

As a person you must be driven, caring, commitment and thrive on exceptional client service. You will have an ability to build and develop great working relationships. You would need to be able to manage multiple priorities while being able to provide great client service, which is key to your success in this role.

**On a daily basis, you will**:

- Effectively and seamlessly allocate referrals to ensure that NDIS participants are provided appropriate services promptly
- Work closely with all NDIS stakeholders to convert enquiries to referrals whilst maintaining exceptional client service
- Provide guidance, support and advice to Participants and others regarding our services at Always About Me (AAM) and the broader scheme of the NDIS
- Work collaboratively with operational team members and leaders to ensure a positive intake and service delivery experience from start to finish
- Manage new and existing funding requests to ensure NDIS funds are available for service
- Provide high quality and individually tailored advice to participants who are navigating the NDIS
- Meet clients face to face and families as required
- Ensure robust case management activity with all activity entered into the system, ensuring all progress, plans, communications and interactions with participants and stakeholders are clearly documented within AAM's policies and procedures and internal processes.
- A solid understanding of the person-centered approach, the principles of the NDIS and the National Disability Service Standards, with experience working within the NDIS and/or disability sector
- Experience in a similar role (desirable)
- Case management or related experience
- Exceptional customer engagement skills and experience, delivering high quality, individually tailored service
- Highly developed communication and negotiation skills
- Ability to work independently, set own priorities, work to targets and respond to a changing environment and meet KPI's.
- Ability to understand and work within prescribed systems and procedures
- Ability to manage IT systems and portals such as PRODA & NDIS commission portal where required.
- Full WA Car Driving Licence
- NDIS workers screening check and NDIS modules completed
- Relevant qualifications to the role.

**Salary**: $60,000.00 - $64,000.00 per year

Supplemental pay types:

- Annual bonus
- Overtime pay
- Penalty rates

Ability to commute/relocate:

- Perth WA: Reliably commute or planning to relocate before starting work (required)

Work Authorisation:

- Australia (required)

Willingness to travel:

- 50% (preferred)
