As a dedicated and compassionate NDIS (National Disability Insurance Scheme) provider and Veteran's Home Care provider Altido plays a pivotal role in enriching the lives of individuals who have served our country and their families and those living with disabilities.

**About the Role**:
We are seeking a dedicated and compassionate **Service Coordinator** to join our team who will be working across 5 days. This role focuses on providing high-quality coordination of care services to veterans, ensuring they receive the support they need in their homes. As a **Service Coordinator**, you will play a key role in coordinating services and working closely with veterans and their families to ensure the best outcomes. You will be responsible for maintaining clear communication with care providers, tracking progress, and resolving any issues that arise during the service delivery process.

**Key Responsibilities**:

- Coordinate the delivery of home care services for veterans, ensuring that all services meet the required standards.
- Provide ongoing support, monitoring, and reviews of the care services to ensure quality and efficiency.
- Act as the primary point of contact for veterans and their families, responding to inquiries and addressing concerns.
- Maintain accurate records of service provision, service plans, and progress reports.
- Ensure compliance with relevant policies, procedures, and regulations, including privacy and confidentiality requirements.
- Maintain a good relationships and communication with Veterans Home care Assessment Agencies and Veterans Home Care Contract Management.

**Key Requirements**:

- Previous experience in the healthcare or community services sector, preferably in aged or veteran care.
- Strong organisational skills with the ability to manage multiple cases and priorities.

- Ability to work under pressure and prioritise tasks to ensure timely action on service plans and support services.
- Excellent communication and interpersonal skills, with a customer-focused approach.
- Ability to work autonomously and as part of a team.
- A qualification in health, social work, or a related field is desirable but not essential.
