**About the opportunity**

We’re currently recruiting for a part time / full-time permanent NDIS Support Coordinator to join our team located at our head office in Carlton. This will be a hybrid role (depending on the hours).

As a NDIS Support Coordinator, you will partner with our clients and their families, and/or support networks to assist them to understand, implement and coordinate their NDIS Plan through a person-centred service which optimises the Client’s NDIS plan and funding. There will be an emphasis on closely with individuals to identify their goals and support them to access the services they need - ensuring the client's choice, control and independence is at the heart of what you do.

This role is responsible for assisting Participants with complex needs to build personal capacity to make decisions and choices about the implementation of all supports in their NDIS plan and focus on addressing barriers and reducing complexity in the support environment, whilst also assisting Participants by ensuring they are on track to achieve their goals as articulated in their NDIS plan or Care Plans.

**Tasks & Responsibilities**:

- Establish rapport and build strong working relationships with clients and support them to develop an understanding of their plan and plan options.
- Assist clients, families, carers, and nominees to understand their rights and build their confidence and abilities to make informed decisions.
- Coordinate services in Participants’ NDIS plans including the management of complex multi-disciplinary teams.
- Assess the funding and services which are available for the client.
- Work with the client, and their support network, to obtain quotes and assessments to meet the elements in the plan and provide maximum value.
- Work with the client to create a schedule of services which support clients to achieve their goals and plans.
- Collaborate with NDIS, mainstream, and existing or potential supports in family, carers, and nominees to provide clients with options for linkages, referrals and supports for clients.
- Regularly review and refine plans with clients and report outcomes to the NDIA through progress reports within the specified timeframes.
- Monitor plan outcomes and expenditure to ensure the services remain within the plan budget for participants.
- Independently manage a varied caseload of participants in the community ensuring quality of care and meeting client objectives and requirements.
- Researching and referring participants to relevant services when required with a focus on improving their health and wellbeing.
- Ensure participants are working towards achieving their goals and being able to clearly document progress in reports.

Qualifications and Experiences:

- Previous experience in the NDIS Support Coordinator Role - Required
- Tertiary Education in Disability, Social Work, Community Services - Desired

Mandatory Requirements:

- NDIS Worker Screening Check - Clearance.
- NDIS Worker Orientation Module.
- Clear Victorian Working with Children Check - Employee status.
- Current CPR and First Aid Certificates.
- Current valid Driver’s Licence and have access to a reliable motor vehicle for work purposes.

Desirables
- Experience with person-centred practice.
- Understanding of case management practices and principles
- Experience liaising with external service providers such as GPs, allied health and other community-based services and organisations.
- Demonstrated ability to identify, measure and report on outcomes.
