**IDS Support Co-Ordinator**
**IDS, established in 1986, is an incorporated not-for-profit registered charity, supporting clients across Melbourne as a registered provider with NDIS, TAC and WorkSafe is looking for a full time Support Co-Ordinator to join its growing team in Carlton, Victoria. IDS share a commitment to offer our clients choice in their support services, assisting them to live as they choose.**
**The Role**
The position is responsible for providing an individualised and responsive support to clients and families in line with the relevant NDIS support plan and legislation. The role is to promote empowerment, choice and control as well as community inclusion of people with disabilities and their families. This includes travelling to engage with our clients and stakeholders where required.
**The Day to Day**
v Meet or exceed Coordination of Support targets and ensure that customer processes are carried out efficiently and within the required timeframes.
v Support a caseload of customers to increase their capacity to understand, monitor and manage supports of their choice.
v Act as the first point of escalation for clients, sharing knowledge, information and expertise with other team members and stakeholders.
v Submit relevant reports to the NDIA in a timely manner.
v Maintain complete case notes and records of hours of support provided to substantiate NDIS claims.
v Develop and maintain effective networks with the other disability and mental health services, other non-government agencies and NDIS.
v Be mindful and adhere to mandatory reporting requirements eg child safe policies and abuse and neglect of adults.
v Increase customers’ engagement in the planning and delivery of support, promoting choice and providing exceptional experiences.
Required Attributes:
- Experience working with people with varied disabilities.
- An understanding of case management practices and principals.
- Successful experience of support coordination for minimum 12 months in coordinating responsive, individualised support to participants with complex needs and their families.
- Demonstrated skill in establishing empowering and supportive partnerships with participants and their families with complex needs.
- Knowledge of the current NDIS, Disability Act and other relevant legislation
- Knowledge of the current trends in service delivery in the community sector, particularly the changing landscape of the disability sector.
- Knowledge of the NDIS Standards, Operational Rules and Guidelines.
- Highly developed communication (both oral and written) skills with the capacity to prepare reports and correspondence in clear and concise language and the ability to assimilate information from other sources, to present information in a manner appropriate to the purpose and audience.
- Ability to lead meetings and emotional intelligence skills, with demonstrated ability to engage others, influence organisational culture, and provide strong direction.
- Well-developed organisational skills, the ability to manage time effectively and prioritise tasks.
- Demonstrated ability to identify, measure and report on outcomes.
- Experience and skills to work competently alongside NDIS participants, their family and carers within their local community to build their capacity to participate in the community and manage some or all aspects of their NDIS plan.

To be considered for this role, you must have a ‘client’ and ‘outcomes’ focused approach and the passion to make each client experience ‘Awesome’.You will have a comprehensive and demonstrated understanding of the NDIS, as well as experience in workforce scheduling or rostering within the disability or aged care sectors.
