**Role Overview**:
We are seeking a motivated and experienced Male Support Coordinator (Bilingual - Greek). You will work closely with clients to support them in navigating and maximizing their NDIS plans. You will ensure they receive the services they need, advocate for their goals, and help them live their best lives.

**Key Responsibilities**:

- Assist clients with planning, understanding, and implementing their NDIS plans.
- Coordinate and manage a range of services, supports, and community resources based on individual client needs.
- Develop personalized service agreements that reflect client goals and preferences.
- Facilitate communication between clients and service providers to ensure smooth service delivery.
- Regularly monitor and review NDIS plans to ensure they are aligned with client goals and support requirements.
- Advocate for clients to ensure they receive the highest quality of care and services.
- Maintain up-to-date records, client progress reports, and service agreements in compliance with NDIS regulations.
- Provide information and guidance on navigating the NDIS system, including funding allocations and service options.
- Ensure the efficient use of client funding while adhering to NDIS budget and requirements.
- Regularly meet with clients (virtually) to check in on their progress and address any concerns or changes in their support needs.
- Identify any risks and implement strategies to mitigate them.
- Uphold confidentiality and professionalism in all client interactions.
- Address complex barriers affecting clients' ability to access the right supports.
- Design and implement service plans for specific support needs, working closely with clients, their families, and carers.
- Work with clients, families, and carers to overcome challenges and set up processes to manage these barriers.
- Develop service plans that facilitate effective communication and cooperation among all support parties and manage problems or unexpected situations.
- Assist clients in understanding when formal advocacy might be needed and support them in seeking this advocacy if required.

**Monthly KPIs (Key Performance Indicators)**:

- Successfully coordinate and oversee services for clients.
- Conduct and document at least 2-3 virtual client check-ins per client monthly.
- Ensure 100% accuracy and timeliness in reporting and maintaining client records.
- Achieve a client satisfaction rating of 85% or higher through regular feedback surveys.
- Maintain adherence to NDIS funding budgets and ensure no misuse or underuse of client funds.
- Develop and review service agreements with clients and providers, ensuring that 100% of plans are updated and signed off before deadlines.
- Identify and implement at least 2 new service providers per month to broaden client options.

**Qualifications**:

- Must be a bilingual. Speaks Greek and English.
- Proven experience in epilepsy management.
- Proven experience as a Support Coordinator or in a similar role within the NDIS sector.
- Strong understanding of NDIS processes, funding management, and service provider networks.
- Exceptional organizational, problem-solving, and time-management skills.
